Requests for Student Records

Disciplinary information is managed by the Office for Community Standards (OCS). Records in OCS are confidential under FERPA, a federal privacy law. Except in very extreme circumstances, our office needs a student’s permission before sharing disciplinary information with someone outside the University. To request a Student Conduct Check, please follow the instructions below

Student Requests

If you are a student and need disciplinary information sent to an external entity, please complete the Request for Disciplinary Information - External form. If there is a form that needs to be complete, please include it in the “Supporting Documentation” section of the form.

Staff Requests

If you are a staff member and a student contacts you about sharing disciplinary information, please complete the Request for Disciplinary Information - Internal form. The requested information will be sent to the appropriate college contact.

External Requests

If you are an external entity requesting disciplinary information, please complete the Request for Disciplinary Information - External form. You must include an Authorization to Release Information form signed by the individual you are requesting disciplinary records for.
